		<description>I don&#039;t believe there is language in any order that says that the new form is specifically &quot;required.&quot; And while Rule 4.01 states what is supposed to be in the Summons, I wouldn&#039;t want to take a chance on a defense of bad service if I used my own form or the old form. Whether a challenge to ineffective service of process is successful, based on using the wrong form of Summons, is another question on which I&#039;m not willing to put a wager.</description>
